Incident Narrative

An employee was unrolling sheet steel. He told a maintenance employee that the stop needed to be adjusted. As the maintenance employee opened the stop gate, it caused the air cylinder to become de-energized. The cylinder then contacted the employee's thumb, fracturing it.
