Incident Narrative

Employees were attempting to get a mill turning after it jammed due to ice. The side was opened and debris was removed. A pipe wrench approximately 2-3 feet long was put on the mill shaft. The injured employee stood on the handle of the wrench to apply force. The wrench slipped off the shaft and the employee fell approximately 2.5 feet onto the mill platform, resulting in fractures to their right ankle.
