Incident Narrative
An employee was using a rag to wipe the side of a moving mixing bowl. The bowl is driven by a horizontally-mounted wheel. While wiping the bowl, the rag the employee was using became caught between the bowl and the wheel, pulling the employee's hand into the space between the two, resulting in a partial amputation of their right middle fingertip.
