Incident Narrative
On January 19, 2024, an employee was operating a motorized pallet jack and was passing another employee on a pallet jack. The employee's pallet jack struck the pallet on the other vehicle. As the employee went to move to the left, the vehicle struck the bumper guard on the floor and the employee's left foot was crushed. Three of the employee's toes were amputated and two were fractured.
