Incident Narrative
An employee was using a forklift to dump waste from a small hopper to a bigger hopper. He elevated the hopper, got out of the forklift, and used a lever to lower the hopper to dump the waste. After dumping the hopper, it swung back and pinched the employee's left index finger between it and the forklift fork, amputating the tip.
