Incident Narrative

An employee was delivering a skid weighing 399 pounds. The employee was sliding the pallet back a few inches so the customer could use their forklift to offload from the ground when the employee slipped and fell out of the back of the trailer. The employee fell to the ground and sustained a fractured left clavicle, rib fractures, and a left parietal scalp hematoma.
