Incident Narrative
On January 13, 2024, an employee was attempting to shut off a valve while standing on a 3-inch pipe that was elevated above the ground. As he turned the crescent tool to close the valve, he slipped and fell to the ground landing on his back. Additionally, three fingers on his left hand were crushed in the crescent tool. The employee suffered a fractured left ring finger, swelling in his back, and a bruise on the right side of his lower back.
