Incident Narrative
An employee was loading sheet metal coil into a metal stamping machine. He was trying to adjust jammed metal that was being fed through the machine's straightener, picking it up with his left hand to help thread it into the die, when the pressure on the metal released. This caused it to shoot forward a few inches, and the employee's left index finger was pinned between the corner of the material and the edge guide. He suffered an amputation to the fingertip.
