Incident Narrative

An employee drove a concrete mixer truck under a plant to be loaded. As the truck turned a corner, the chute on the side of the truck became wedged against a wall; this interfered with the spinning of the barrel. The employee got out of the truck to free the chute from the wall. In the process, he suffered severe lacerations to his fingers.
