Incident Narrative
On December 19, 2023, an employee was working from a 6-foot ladder decommissioning an old remote cabinet when the rear bolts broke loose. The remote cabinet knocked the employee off the ladder and he fell to the floor. The 30-pound remote cabinet then fell on the employee, causing fractures to the left wrist and left side rib cage.
