Incident Narrative
An employee cleared a potato shredder machine that had a bag and potatoes stuck in it, causing them to spill on the floor. The employee restarted the machine and was cleaning up the potatoes on the floor when he slipped on a potato and fell backward. His right then struck the machine and his fingers went between the chain guard and the frame. His index finger was amputated to the first knuckle.
