Incident Narrative
An employee was using a step ladder to access a mezzanine in the warehouse. Once the employee stepped off the ladder onto the mezzanine, the ladder fell from underneath the employee, causing them to fall 6 feet to the concrete floor. The employee sustained a fracture to the bottom of their right foot and a sprained wrist.
