Incident Narrative
An employee was putting together a trench box and was aligning the spreader bar with the shields. One side was pinned and the employee was working on the other side when the machine went to move it. Both spreader bars fell and one crushed the employee's right foot. The employee was hospitalized and required amputation of the foot.
