Incident Narrative

An employee was on a vehicle loosening the tie downs on the aluminum coils he transported to a customer location. As he was walking around the coil, he pulled on a strap for balance. The strap he pulled on was loose, causing him to lose his balance and fall to the ground, landing on both feet. The employee suffered fractures in both ankles.
