Incident Narrative
An employee cleared dust accumulation from the exhaust fan blades of a pellet cooler before cleaning the pellet cooler itself. As they were closing the inspection hatch to the airlock, the fan blade contacted three fingers on their right hand, resulting in a laceration to the index finger and fingertip amputations of the ring and middle fingers that required further amputation past the nailbed.
