Incident Narrative
An employee was placing base plates on the floor when he noticed another base plate falling off the deck. As he reached for it, he fell through a guardrail system on the second story deck to the dirt ground below. The employee sustained fractures to the nasal bone as well as vertebrae in the cervical, thoracic, and lumbar regions. The employee also suffered fluid in his lungs.
