Incident Narrative
An employee was cleaning a mixer and using pressurized water to spray out debris. The mixer was running and the agitator shaft was rotating. As the employee sprayed the moving parts, their right hand became caught and was pulled into the front discharge area resulting in amputation of the middle fingertip.
