Incident Narrative
An employee was pressure testing a 6-inch water line. To relieve the pressure, the employee loosened one side of the flange fitting on the pipe. Pressure in the pipe caused the flange to fly off the pipe. The flange didn t strike the employee, but rust and debris that had built up around the flange and inside the pipe struck the upper right side of their face and eye, resulting in lacerations and bruising.
