Incident Narrative
On October 14, 2023, employee 2 was moving a conveyor for cleaning. The conveyor had become stuck on a deflection tray of a slicing machine. As employee 2 was pulling the conveyor away, the injured employee went to move the deflection tray out of the way when their right fifth finger became caught between the tray and the side of the conveyor, resulting in partial amputation.
