Incident Narrative
A maintenance employee was on top of an elevator car top and had taken manual control of it using the top of car inspection station. The employee was moving the elevator in upward when a roll of reflective tape in his right hand got caught between the elevator car and the elevator shaft wall. The roll of tape twisted, crushing the employees right little finger. The finger was fractured and lacerated.
