Incident Narrative
Employees were changing a HEPA filter on top of a pre-weigh booth. The injured employee was unable to reach the housing from a scissor left. He went onto the roof of the booth to unscrew the housing from the old filter. When the injured employee shifted his weight, the roof collapsed. He fell to the floor inside the booth and sustained fractures to his wrists and hips.
