Incident Narrative

An employee was pulling tubular welded scaffold frames from a storage area and loading them onto the forks of a forklift. The frames were sitting on ground that was muddy and a few frames fell and knocked the employee over. He fell onto a raised concrete manhole cover and sustained three broken ribs and a possible back injury.
