Incident Narrative

An employee was staging pallets using a powered industrial truck (PIT) and a racking system. The employee was backing out from under the racking system when the vehicle struck the rack and was caught between it and the PIT. The employee suffered two lacerations to their left shin, requiring hospitalization and stitches.
