Incident Narrative
An employee was transferring a heavy ammo box from a dolly to the delivery vehicle. He struck his knee cap. While re-entering the building, his knee buckled. While he was bracing for a fall, the second entry door of the receiving area closed on his left little finger removing flesh from the finger.
