Incident Narrative

An employee had been waiting for tubing to be lowered. The swage came loose and dropped along with the tubing (approximately 31-feet long). It fell 60 feet to the ground, then tipped over and struck the employee's head. The employee was knocked out and sustained a concussion, a broken nose, and lacerations to the head.
