Incident Narrative

An employee was operating a leveling machine while filling a bed with mushroom compost. The machine became jammed from a rock in the compost. To clear the jam, the machine was shut down and the guard was removed. As the employee went to remove the rock, the machine activated, resulting in a right arm laceration with exposed bone and an injury to the right shoulder, elbow, and hand.
