Incident Narrative

An employee was making a repair on the chain/sprocket of the high-speed door of a freezer when the door was activated from inside the freezer. As the door raised, the drive chain assembly caught the employee's middle, ring, and little fingers on his right hand between the chain and sprocket resulting in fingertip amputations. The employee was hospitalized.
