Incident Narrative
A temporary employee was operating a walkie rider pallet jack. They were coming out from between two racks when their pallet jack made glancing contact with another walkie rider pallet jack, then struck the rear of a forklift. The employee sustained a fracture to the right medial malleolus, four dislocated toes on the right foot that required surgery, and a laceration to the right foot.
