Incident Narrative

An employee was installing insulation in a second-floor wall from an A-frame ladder that was leaned against the wall he was working on. While positioning himself to the right side of the ladder, the ladder shifted, causing the employee to fall approximately 8 feet. He landed on the ladder, then the floor, resulting in broken ribs.
