Incident Narrative
A crew was changing out a 3-by-3 studded block cross on a stand. They had used a forklift, forklift hooks, and straps to lift the stand and block using the jack handle arms as lifting points. While the U-bolt nuts were being tightened, the straps slipped, the stand tilted, and the block slid off the stand and landed on an employee's left leg and arm. The employee suffered a broken left femur, left ulna, and left radius.
