Incident Narrative

An employee had just completed inspecting an incoming load of material. The employee went to a processing bay and used an industrial shear to cut copper. While reaching for additional material to cut, the upper jaw of the shear was activated, causing the shear to close on his fingers. Two fingers were partially amputated.
