Incident Narrative
The injured employee was assisting employee 2 prepare a boiler for inspection by removing the hand hole cover to access the water side of the boiler. Employee 2 had the cover held with pliers to prevent it from falling into the boiler while the injured employee was loosening the bolts to the cover. Non-pressurized hot water flowed from the opening and contacted the injured employee, causing third-degree burns to 37% of his body on the left side of his torso and legs. He was hospitalized and had surgery.
