An employee had been monitoring the progression of pipe onto a reel. The employee was removing a section of chain that restrains an air plug (which is inserted into the flexible pipe as part of the process), when the chain became hung up on the rotating take-up reel. The employee was untangling the chain when they sustained an amputation to their right thumb tip.
