Incident Narrative
An employee was troubleshooting a palletizer. After completing maintenance, the employee was re-aligning the chain drive with another employee. The employees were jumping the chain and using a prybar to align it on the sprocket. The pry bar slipped and the injured employee's left middle finger was caught between the chain and sprocket, resulting in a fingertip amputation.
