Incident Narrative

An employee was experiencing a mechanical issue with the fan belt on his roll-off truck and called a tow truck. The tow truck lifted the front wheels of the truck approximately 18 inches off the ground and the driver asked the employee to release the roll-off truck's parking brake. As the employee exited the cab, he fell onto the concrete ground, resulting in a right hip fracture and loss of consciousness from striking his head.
