Incident Narrative
On July 19, 2023, an employee stepped into the passenger seat of a forklift that was turned off to speak to the forklift operator. The employee's right hand was on the forklift's casing when the forks moved and his right middle and ring fingers were pinched between the housing unit and the forks. His middle fingertip was surgically amputated with bone and nail loss.
