Incident Narrative
An employee was installing support bars on a gin pole truck when the hydraulic cylinder moved and struck the employee. This caused the bars to fall and strike the employee. The employee then fell onto the concrete floor, sustaining fractures to a rib, vertebrae, and the pelvis, as well as a head contusion.
