Incident Narrative

An employee and a co-worker put a 16-foot extension ladder on top of a baker scaffold to paint on a wall in a foyer approximately two stories high. As the employee was standing on top of the ladder, it shook the scaffold and the ladder then toppled over, causing the employee to fall head first onto the tiled floor and sustain a head injury.
