Incident Narrative

At 2:00 AM on June 23, 2023, an employee was using a torque gun to loosen bolts on a piece of equipment. While positioning the torque gun onto a bolt, the trigger on the torque gun was pressed. The metal arm of the torque gun moved and struck the employee's finger, causing an open fracture and amputation of his right middle fingertip.
