Incident Narrative
Employees were cleaning a storage tank for petroleum. Employee 1 was inside the tank and employee 2 was outside the tank approximately 30 to 40 feet away when a flash fire occurred. The force of the fire/explosion threw employee 1 into a wall. Employee 1 sustained fractures to the left collar bone and left ribs, a dislocated right elbow, a lacerated left ear, and burns. Employee 2 sustained second-and-third- degree burns to the face, chest, hands, and fingers.
