Incident Narrative
An employee was using a forklift to offload a channel from a truck while another employee was standing near the vehicle observing. The channel on the forklift became unsteady and the load began to fall, striking the employee that was observing. He was hospitalized for a leg fracture and injuries to his left elbow and side that required surgery.
