Incident Narrative

An employee was pouring concrete into a 400-foot-long soffit. The employee cut it to the right length and put cables to hold it in place in the prestress bed. He also laid 3 feet of cable behind the soffit. As the employee held onto a piece of cable, it popped out. He let go of the cable and reached inside the soffit. The employee's right index fingertip was pinched between the steel strand chucking device and a stationary steel abutment. The employee suffered an amputation to the right index finger.
