Incident Narrative

An employee was using a forklift to unload aluminum rectangular tubing onto storage racks. The tubing became stuck and the employee got off the vehicle to manually unload the tubing from the forklift. The tubing ricocheted off a storage rack, struck the bottom of the forklift, and then the employee's left hand resulting in a middle fingertip amputation.
