Incident Narrative

An employee was operating a machine that wound polyester yarn through a piece of hollow sleeving. The in-feed yarn became caught under the spool on the floor. The employee bent down to the floor and tried to clear the yarn from under the spool. When he stood up, the yarn whipped and brushed against his left arm, then entangled his left thumb. It then pulled his thumb against a guidepost, where the thumb was amputated.
