Incident Narrative

An employee and a co-worker were loading a 4-foot-tall piece of high-pressure iron pipe into a line truck. When the employee was setting down the pipe, his right middle finger was caught between the hammer union wing and another piece of high-pressure iron pipe, resulting in a finger amputation.
