Incident Narrative

Two employees were rebuilding the hydraulic system for a 14,000-pound forklift. They were removing the hydraulic hose when the hydraulics lost pressure, causing the bucket of the forklift to drop down onto the employees. One employee suffered broken arms and legs, as well as head injuries.
