Incident Narrative
An employee was working on a pellet extruder machine that had a dry ice blockage that needed to be removed. As the employee removed the die head, a buildup of carbon dioxide gas under pressure propelled the 5/8-inch die into the employee. They sustained lacerations to the face, fractures of the nose, jaw, and eye socket, as well as internal head trauma.
