Incident Narrative

An employee was working on a mold on a work bench. They attached an angle plate to stabilize the mold and started to hoist the mold to move it to the floor. The employee was holding onto the angle plate with their left hand when the bolt that was holding the mold failed, causing the angle plate to fall onto the employee's left little finger. The employee sustained a fingertip amputation.
