Incident Narrative

An employee was on an elevated forklift man basket hanging a tarp on the top of a stack of bins to provide shade to the outdoor work area. A gust of wind pulled the tarp, causing the man basket to slide on the forks. The man basket then slid off the forks and the employee jumped out of the basket as it fell, resulting in a closed fracture of the T12 vertebra and a fractured left big toe.
