Incident Narrative

Two employees were cleaning out a road widener. The injured employee was standing in front of the road widener using a hose to clean out concrete from the equipment. The seat in the cab turned and bumped a lever, causing the road widener to move forward. It knocked the injured employee to the ground and then rolled over him. The employee was hospitalized with a broken ankle.
